
Prescription Refill Policy - Prescriptions will not be refilled if called in on weekends, holidays or after 4pm Monday - Friday. If you have not been seen in the office for over 3 months, you must make an appointment to see the doctor before a refill will be considered. We are happy to work with mail-in pharmacy services.
Test Scheduling Policy - Our office will precertify your test, if your insurance requires it. We can schedule the test for you at a facility covered by your insurance company. Once the test is complete, please allow 3-5 business days for us to obtain the results. You must make an appointment to review the results with the doctor in person.
Medical Records Policy - Under Federal and Ohio law, material found in the medical record is generally considered confidential between the physician and patient. Every effort will be made to protect the rights of the person with regard to medical record information and disclosure of medical information. A written and dated authorization by the patient may be requested of the individual inquiring about patient information before such information is released.
Patients needing copies of x-rays in our possession should call 513-791-6611 and ask to speak to someone in the X-ray Department.
Depending on the nature of the request, there may be a fee for copying and mailing patient records or x-rays.
